Understanding Bulging & Herniated Discs

Spinal discs act like cushions between the bones of the spine. When a disc becomes damaged or changes over time, it may bulge, tear, or herniate and sometimes irritate nearby nerves.

What Is a Bulging or Herniated Disc?

Intervertebral discs are located between the vertebrae of the spine. They provide cushioning, absorb pressure, and help the spine move comfortably.

Over time, discs naturally lose some of their water content and flexibility. This can make them more vulnerable to changes such as bulging, tearing, or herniation. Disc problems may also develop after an injury.

When a damaged disc irritates or places pressure on nearby nerve roots, it can cause pain that travels beyond the spine. In other cases, the disc itself may be a source of localized neck or back pain.

A bulging or herniated disc seen on an MRI does not automatically mean that disc is causing your pain.

How a Disc Can Cause Pain

Disc-related symptoms generally happen in one of two ways: irritation of a nearby nerve or pain coming from the disc itself.

Disc Degeneration

The disc may gradually lose hydration, flexibility, and cushioning as it changes over time.

Bulging or Herniation

The disc may extend beyond its normal position or develop a tear that allows inner disc material to move outward.

Nerve Irritation

Disc material or inflammation may irritate or compress a nearby nerve root.

Radiating Symptoms

Irritated nerves may cause pain, tingling, numbness, or other symptoms that travel into an arm or leg.

How Do I Know If I Have a Herniated Disc?

Spine terminology can be confusing. You may hear phrases such as herniated disc, bulging disc, slipped disc, or pinched nerve. These terms may describe related problems, but they do not always mean exactly the same thing.

Symptoms depend on where the affected disc is located and whether nearby nerves or other structures are involved.

Neck or lower back pain Sharp or shooting pain Pain traveling into an arm or leg Numbness or tingling Muscle weakness Pain with certain movements or positions

Which Discs Herniate Most Often?

Herniated discs most commonly affect areas of the spine that move frequently and carry significant mechanical stress.

Cervical Spine

Herniated discs in the neck may cause neck pain or symptoms that travel into the shoulder, arm, or hand.

Lumbar Spine

Herniated discs in the lower back may cause back pain or symptoms that travel into the buttock, leg, or foot.

Diagnosis

Finding the True Source of Your Pain Comes First

A diagnosis is based on much more than an MRI. Your provider will consider where your pain occurs, what the pain feels like, what makes it better or worse, and whether there are signs of nerve involvement.

Detailed medical history

Physical examination

Movement testing

Muscle strength testing

Reflex testing

MRI or CT imaging when appropriate

Additional diagnostic testing may sometimes be recommended depending on your symptoms and the suspected source of your pain.
